In April 2012, International Risk Management Institute’s The Risk Report published Kristina Narvaez’s article on “Using Root Cause Analysis in Enterprise Risk Management.”
RCA is part of the risk assessment process of an ERM program. It is a structured process designed to help an organization define causes to past or future risk events, understand the causes of those risk events, and most importantly, prevent future incidents from happening. Case studies of the Macondo Well Explosion and Bonneville Power Administration are discussed in this article.
